Facility Standards

Comprehensive Scope of Services

Mountain Paradise Care Home provides a complete continuum of care certified under Arizona state assisted living regulations.

Levels of Care

  • Supervisory Care: General supervision, daily functioning awareness, crisis intervention, and self-administration medication support.
  • Personal Care: Hands-on assistance with ADLs (dressing, grooming, hygiene) supervised by a licensed nurse.
  • Directed Care: Specialized programs for residents unable to recognize danger or make basic care decisions.

Medical & Health Care

  • Medication & Diabetic Care: Certified caregiver administration, glucose testing (FSBS), and insulin supervision per practitioner orders.
  • Health Monitoring: Daily health tracking, medical record maintenance, and continuous health reviews by a Licensed Nurse.
  • Therapy & Home Health: Coordination for Physical/Occupational Therapy, Wound Care, and Catheter Care via outside agencies.

24-Hour Staffing & Safety

  • 24/7 Certified Staff: On-premises Certified Manager and Caregivers with 2-hour checks during night hours.
  • Bedside Alert Systems: Bedside monitors, call bells, or alerting devices for immediate caregiver assistance.
  • Secured Environment: Alarmed front door security to maintain safety without creating a lockdown facility.

Dietary & Nutrition

  • Nutritious Meal Plans: 3 pre-planned daily meals and snacks following the Dietary Guidelines for Older Adults.
  • Custom Food Preferences: Special modifications to diet textures and feeding assistance when needed.
  • 24/7 Snack & Coffee Station: Fresh coffee and snacks always accessible to residents and guests.

Housekeeping & Laundry

  • Daily Cleaning: Basic daily housekeeping for all rooms and weekly deep cleaning including outdoor patio areas.
  • Personal Laundry: Weekly personal laundry service and bed linen changed 3x per week or as needed.
  • Bathing & Grooming: Assistance with showering, hair care, skin lotion, nail care, and incontinence care.

Recreation & Hospice

  • Daily Social Activities: Reading current events, games, puzzles, cognitive stimulation, and gentle exercises.
  • Hospice Agency Services: Compassionate end-of-life care in coordination with outside hospice medical teams.
  • Annual Testing: Facilitation of annual influenza immunizations and Tuberculosis (TB) testing.
